Management staff assigns the <br /> site a "D" status, if applicable and assigns a LOP staff to the site. <br /> S/61/4 CasteA��ic�yi •�� Gabriel Ganz.a/ei . <br /> Clerical staff enters new site information using the Site Mitigation Form completed by <br /> staff into Envision at Site Remediation and Case Information records, through Envision <br /> creates new RO number and a new case alias number using the RO number (omit "RO" <br /> and enter all digits of the number), then creates a new LOP site file. From Oracle <br /> Application Services Website, LOP staff print screen shots of "owner history" and <br /> "property information" and file in new site file. Clerical staff notifies accounting of the new <br /> LOP site. All a//g/// <br /> 5 9 / Assigned LOP Staff reviews Unit III site file for history, UST an soil stock pile disposal <br /> information, obtains copies of soil sample analysis, maps, Prop 65 completed by Unit III <br /> staff, a signed "Authorization to Release" document; and completes Site Mitigation <br /> Record Form for new LOP site file. <br /> 31A, LOP Staff verifies who the Responsible Party(s) are. There may be more than one if the <br /> tank owner is different from the property owner. From Parcel Viewer, print screen shot <br /> of site parcel to document/verify property owner, APN and location of the site. <br /> 5--q,14 Contact CVRWQCB (Jim Barton) for a new LOP site number (36000) for GT. <br /> SJ1(.-19 1vAt the Geotracker Website the LOP Staff: <br /> submits an Unauthorized Release Form (URF), by entering new LOP site <br /> information at `Creates a New Project' screen. Use RO number for the Case <br /> Number (omit "RO" and enter all digits of the number), <br /> Makes site specific entries at all applicable site screens to keep site up to date <br /> with required reporting (`Closure Review, `Project Information', `Groundwater <br /> Monitoring Frequency', etc.). Be sure to update `Activities Report' page with a <br /> currently dated activity. <br /> ��-V L P Staff sends "Notice of Responsibility" and "Site Letter" to RP(s) that notifies them <br /> their site has been assigned to the EHD Unit IV LOP. Letter is copied and sent to the <br /> CVRWQCB, and the SWRCB UST Section and Unit III. "Dirty Letter" is no longer sent. A <br /> Site letter is sent instead. (if Unit III asks), <br /> LOP Staff sends a directive letter to the RP(s) to submit a work plan to begin and <br /> investigation of the lateral and vertical extent of the contamination. - Note: this directive <br /> may be included in the Site Letter <br />
