I informe <br /> Moorman's that I had not received notification. Moorman's representedl Je- <br /> informed me that he was sorry but he had not been informed by the on-site <br /> crew that additional notification was requested. Discussed further <br /> scheduling. No schedules were confirmed. <br /> Discussed Moorman's perforation without inspection-and the equipment which <br /> was on-site with PHS/EHD staff. <br /> Phone call to the City of Stockton regarding perforation without <br /> inspection. Left message. <br /> Phone call from the City of Stockton. Discussed perforation and equipment <br /> available. A site meeting was tentatively scheduled later that afternoon <br /> with the City of Stockton, Moorman's and PHS/EHD to discuss possible re- <br /> perforation under PHS/EHD inspection. <br /> Numerous phone calls from Moorman's to discuss PHS/EHD possible request to <br /> re-perforate. <br /> Site visit. Arrived on-site at approximately 3:30 ftvx Discussed equipment <br /> available and communications.
