Please see attached photos. <br /> #60: Attached, please find the signed copy of manifest number 000461171 FLE <br /> (5/7/09). Typically manifested waste is shipped via Clean Harbors and falls <br /> under the Clean Harbors electronic manifest tracking system; however, this <br /> particular manifest was an exception — it contained a drum of PCB lighting <br /> ballasts which was picked up by AERC Recycling. When the signed copy was <br /> received at our administration office we overlooked getting a copy back to the <br /> facility(and since an outside vendor was used, this manifest did not fall under the <br /> Clean Harbors electronic tracking system). A signed copy is now at the facility. <br /> #61: Upon receipt of the signed manifest number 000461171 FLE, our office <br /> overlooked sending a copy to the facility (see #60). A copy is now at the facility. <br /> #62: Again, this was an administrative oversight. Waste from manifest number <br /> 000461171 FLE was received and signed off by the receiving facility on 5/7/09; no <br /> exception report is necessary as the waste was properly managed.
