z , <br />SAN:JOAQUIN o R' <br />—COUNTY-- a <br />�5„�” Greatness grows h.: <br />Woking for YOU <br />May 11, 2017 <br />Ms. Elena Manzo <br />Environmental Health Department <br />1868 East Hazelton Avenue <br />Stockton, California 95205 <br />Department of Public Works <br />Kris Bedell, Directorof Public Works <br />Fritz Buchman, Deputy Director/Development <br />Michael Selling, Deputy Director/Engineering <br />Jim Stone, Deputy Director/Operations <br />Najee Zadf, Interim Business Administrator <br />SUBJECT: STATEMENT OF CORRECTIVE ACTIONS — SJC HHW FACILITY <br />Dear Ms. Manzo: <br />The following corrective actions have been taken to address the violations specified in the <br />Permanent Household Hazardous Waste Collection Facility Inspection Report from the <br />June 29, 2016, San Joaquin County HHW Facility inspection: <br />402) Lined Lab Pack cardboard boxes (Clean Earth System Inc., Lab -Pak 55, UN Rated <br />4G/X158.8/s/16/USA+AA6576) are used at the site to store various hazardous wastes. <br />The boxes are used primarily for waste stream UN2929 Toxic Liquid, Flammable, <br />Organic, N.O.S. (Diazanon, Petroleum Distillates) 6.1(3), and PGII. <br />Page one of attached "Clean Earth Design Qualification" is a document from the <br />manufacturer with the specifications. Page two to four highlights compatibility <br />information for the waste stream the boxes are used for.
