The inspection was initiated on 6-29-2016, <br />was continued on 6-30-2016, and was finalized on 7-1-2016. During the inspection, the EHD inspector was <br />accompanied by Ms. Elisa Moberly (PHHWF Managing Analyst) and Ms. Maelee Janvin (011ie, PHHWF <br />supervisor). An inspection checklist was provided to the facility operator at the time of inspection. <br />The EHD has written the complete report which replaces the initial checklist. Complete and submit a copy of the <br />Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that <br />have been or will be taken for each violation, and any supporting paperwork, within 30 days of receiving the <br />complete inspection report. <br />NOTES: <br />1. Due to the tank tampering incidences that this facility had in the past, the existing used oil tanks were <br />cleaned out, certified clean, and not in use at the time of inspection. Reportedly this facility is in the process of <br />procuring a more secure used oil collection tank system. <br />2. According to the reviewed records, this facility also recycled the following wastes in 2015: CRTs- 12.5 tons, <br />other universal waste electronic devises 12.89 tons, 6.44 tons of lithium type batteries, 4.66 tons of lamps, and <br />60 tons of latex paint. <br />An inspection report was provided to the facility operator on the day of inspection. The report was amended by <br />the EHD.
