All cylinders received during the reporting <br />period were determined to be recyclable and transported to the appropriate facility under a <br />bill of lading. No cylinders were improperly identified and shipped. Please retract #105, <br />which is documented as a Class II repeat violation. <br />110) The issue was corrected on site during the inspection; the container of antifreeze was set to <br />drain into the waste antifreeze storage tank. Staff received training to review the procedures <br />for emptying containers, and a visual aid was placed near the tank. Please see attached <br />proof of training and visual aid. <br />114/116) <br />The following manifests had been mailed to DTSC, but the dates were not noted on the <br />summary sheets: <br />• Manifest 004373574 SKS was mailed to DTSC on August 29, 2014 <br />• Manifest 004373782 SKS was mailed to DTSC on October 27, 2014 <br />• Manifest 004298263 SKS was mailed to DTSC on January 8, 2015 <br />• Manifest 004374497 SKS was mailed to DTSC on September 9, 2014 <br />• Manifest 004212473 SKS was mailed to DTSC on October 27, 2014 <br />• Manifest 004351304 SKS was mailed to DTSC on January 8, 2015 <br />• Manifest 004422507 SKS was mailed to DTSC on October 27, 2014 <br />
