Soil samples collected from the <br /> bottom of the tank pits evidenced significant petroleum hydrocarbon contamination upon <br /> analysis. On May 11, 1999 one 300-gallon waste oil tank was removed from the site, and again <br /> contamination was noted. San Joaquin County Public Health Services, Environmental Health <br /> Division (PHS/EHD) attempted to notify you by certified mail dated June 3, 1999, that your site <br /> had been assigned to the Local Oversight Program for oversight of assessment and cleanup <br /> activities at contaminated underground storage tank sites. You were requested to submit a <br /> work plan to begin the investigation of the vertical and lateral extent of contaminant distribution <br /> at your site to PHS/EHD by July 30, 1999. The certified letter was left unclaimed and eventually <br /> returned to the PHS/EHD office. PHS/EHD mailed the correspondence to you by regular mail <br /> on July 8, 1999. <br /> Please submit the previously requested work plan. Include in the work plan a site specific <br /> Health and Safety Plan and a well survey that identifies the location of all domestic, agricultural, <br /> municipal, industrial and other wells within a 2000-foot radius of the site.
