Box 388 • Stockton, CA 95201-0388 <br /> 209/468-3420 <br /> HORST HANF <br /> 834 SAN FRANCISCO BLVD <br /> ' SAN RAFAEL CA 94901 <br /> RE: Tiki Lagun Resort & Marina SITE CODE: 1679 <br /> 12988 West McDonald Island Road <br /> Stockton, CA 95206 <br /> The attached "Notice of Reimbursement" is official notification that your site has been assigned <br /> to the San Joaquin County Public Health Services, Environmental Health Division's (PHS/EHD) <br /> Local UST Oversight Program for oversight of assessment and cleanup activities at contaminated <br /> underground tank sites. <br /> PHS/EHD sent you a letter dated March 8, 1994 advising you that we will be utilizing the State <br /> Water Resources Control Board (SWRCB) "Prioritization System for Leaking Underground <br /> Storage Tank Sites" ranking list in order to prioritize all the contaminated sites. The letter stated <br /> that based upon the priority ranking you will be directed to begin the required investigation. <br /> At this time PHS/EHD is directing you to begin the investigation to determine the impact to soil <br /> and groundwater from the unauthorized release of petroleum hydrocarbons as confirmed by <br /> laboratory analysis of soil samples obtained during the removal of the UST at the above <br /> r <br /> referenced site. <br /> Fees for review activities and inspections associated with contaminated site assessment activities <br /> will not be billed by this office. The State Water Resource Control Board (SWRCB) will <br /> invoice you directly for activities performed under the auspices of the Local UST Oversight <br /> Program. The goal of the site evaluation performed by PHS/EHD is to certify closures of <br /> underground tank leakage sites. Closure certification occurs only after mitigation activities <br /> are completed. <br /> In addition, the "Regional Board Staff Recommendations For Initial Evaluation and Investigation <br /> of Underground Tanks" and "Appendix A" reporting format and the LUFT Manual shall be <br /> utilized as guidance documents in the preparation of work plans and/or reports. <br /> Enclosed please find a list of consultants to contact for proposals for further investigation. <br /> A workplan for this investigation should be submitted to PHS/EHD by May 24, 1995. <br /> A Division of San Joaquin County Health Care Services <br />
