Nack <br /> Site Safety Officer: Allan Jin Fetko <br /> This site safety and health plan (SSP) must be kept on-site and accessible to all ori-site employees <br /> during all phases of site operations. The SSP must be reviewed and signed by all on-site <br /> employees, subcontractors, and visitors prior to entering the site. General safe work practices <br /> identified in the Earth Systems Environmental, Inc. (ESE) Injury and Illness Prevention Manual <br /> apply to all ESE work sites. <br /> The provisions set forth in this SSP shall apply to ESE employees and any subcontractors working <br /> for FSE at the job site. All personnel working for ESE at the lob site must read this SSP,and sign <br /> the attached Compliance Agreement before entering the work area. Field personnel may deviate <br /> from the safety provisions set-forth in this SSP, but only to upgrade or increase the safety <br /> requirements. ESE personnel may suspend work if unauthorized modifications to the safety <br /> provisions set-forth in this SSP are made.
