This should <br /> be the final activity that will allow the San Joaquin County Environmental Health Department (EHD) <br /> to issue a `no fiuther action' letter. Attached are copies of the fully executed permit (SR 59412) for <br /> the work, a site plan showing the well locations, and the three DWR-188 reports (originals were <br /> mailed to DWR). <br /> The well destructions were performed on March 2, 2010 by WDC, a licensed (C57-283326) well <br /> drilling company from Richmond, CA. David Guthridge from LA&S and Mike Infurna from the EHD <br /> were on-site to witness the procedures. First, a neat cement grout(1 bag Portland cement mixed with <br /> 6 gallons of tap water) was injected under pressure into each of the three 2-inch diameter, —22-foot <br /> deep well casings. After the standard pressurization (30 psi for minimum of 3 minutes) to force the <br /> grout into the well pack, each casing was topped off with grout. The next step involved drilling out <br /> the upper—3 feet of each well, including the vault. A plug of cement about 18 inches thick was placed <br /> in the resulting hole. The balance of each hole was filled with the drill-out materials.
