Environmental Health Department <br />Hazardous Materials Program Inspection Report <br />Date: <br />March 16, 2020 <br /> Facility Address: <br /> 1301 W KETTLEMAN LN, LODI <br /> Facility Name: <br /> FLAMES LIQUOR <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br />A site map was not completed or submitted as part of the business plan. <br />- Hazardous material handling and storage areas for propane stored outside the store are not shown. <br />A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, <br />access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage <br />areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the <br />map and label as “NA". Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This was corrected on site. <br />This is a minor violation. <br /> 9 HSC 25508(a)(1) Failed to annually review and certify that the business plan is complete, accurate, and up to date. <br />The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br />System (CERS) annually. <br />- The business plan for the year 2018, 2019 and 2020 were all submitted late, <br /> 2020 submission made on 2/19/2020 - late <br /> 2019 submission made on 3/05/2019 - late <br /> 2018 submission made on 1/22/2018 - late <br />The hazardous materials inventory shall be submitted by January 15 of each calendar year and may be submitted <br />beginning November 1 of the previous year. Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the <br />correct or updated information, and submit to the EHD for approval. <br />This is a Class II violation. <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by April 15,2020. <br />To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br />of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br />($152).
