INTRODUCTION <br /> At the request of Mr Nick Bokides of Mel Bokides Petroleum, Inc ,Advanced GeoEnvlronmental, <br />' Inc (AGE)has prepared the enclosed report for the former Mel Bokides Petroleum facility located <br /> at 444 Mossdale Road,Lathrop,California(site) The scope of work Included gauging and sampling <br /> the on-site ground water monitoring well network, submission of monitoring and analytical data to <br />' the State Water Resources Control Board's GeoTracker Information management system, and <br /> preparation of this report The site setting is Illustrated on Figure 1 Site structures, previous soil <br />' borings, and monitoring well locations are depicted on Figure 2 <br /> Field work was performed in accordance with Central Valley Regional Water Quality Control Board <br />' (CVRWQCB)and San Joaquin County Environmental Health Department(EHD)guidelines for the <br /> subsurface investigation of underground storage tank(UST)sites and ground water monitoring well <br /> sampling procedures, respectively The site history, a description of work previously performed at <br />' the site with analytical data from the previous investigations, and a site conceptual model are <br /> summarized in Appendix A <br /> 2.0. PROCEDURES <br /> Field procedures were performed in accordance with Central Valley Regional Water Quality Control <br />' Board(CVRWQCB) guidelines for the investigation of underground storage tank (UST) sites and <br /> San Joaquin County Environmental Health Department(EHD)guidelines for the sampling of ground <br />' water monitoring wells The procedures were performed in accordance with the AGE-prepared and <br /> EHD-approved Preliminary Site Assessment Work Plan, dated 15 September 1999 <br /> 2 1 4 PURGE WATER DISPOSAL <br /> �J.� <br /> On 2uly 2005,approximately 100 gallons of non-hazardous purge water was pumped from on-site <br /> 55-gallon drums The purge water had been generated during previous ground water monitoring <br />' events Pumping and disposal was completed by American Valley Waste Oil, Inc of Delhi, <br /> California The purge water was disposed at Riverbank Oil Transfer located in Riverbank,California <br /> A copy of the waste disposal manifest is included in Appendix B <br /> 2 2 WELL MONITORING AND EVACUATION <br /> rOn 28 July 2005,a Solinst water level meter was used to measure the depth to ground water in <br /> . monitoring wells MW-1 through MW-5 After depth to ground water measurements were obtained, <br /> Advanced CeoEnvironmental,Inc <br />
