SOP #12 <br /> Date:9/1/93 <br /> Page 23 of 31 <br /> 8.0 <br /> DOCUMENTATION <br /> The field documentation requirements for the ERM-West, Inc., CPT rig supervisor <br /> will include .recording all observations made during probing, sampling, or <br /> decontamination that could affect the quality of a sample. The documentation for <br /> each sample should be entered in a bound field notebook with consecutively <br /> numbered pages or on a data collection form. <br /> Documentation should include at a minimum: <br /> • Date of investigation/location/diagram or map <br /> • Date of sample collection <br /> • Time of day <br /> • CPT ID number <br /> • Depth of sample <br /> • Type of sample (i.e., soil or water, QA/QC sample) <br /> • Sampling device <br /> • Surrounding conditions (i.e., weather, etc.) <br /> • Unusual conditions <br /> • Decontamination procedures <br /> • FID or PID readings <br /> • Generalized log with sample locations, sample numbers, and relevant <br /> observations <br />
