(Federal Express) <br /> Dear Ms. Ratliff: <br /> Enclosed, please . find the necessary documentation and fees for <br /> permitting of 10 + soil borings to be drilled at Stockton Scavenger <br /> Assoc. The drilling constitutes the first phase of a three phase <br /> work plan designed to characterize and define the extent of <br /> groundwater contamination at the site (soil borings will be drilled <br /> to top of the water table and Hydro-Punch apparatus used to extract <br /> a groundwater sample) . The Central Valley Regional Water Quality <br /> Control Board has reviewed and authorized implementation of the <br /> work plan. Their approval letter is included in this submittal. <br /> A copy of the 28 Nov 1989 work plan was forwarded to your agency. <br /> Other documents included in this submittal are as follows: <br /> * Application: Environmental Health/Permit Services <br /> * Application for Permit: San Joaquin Local Health District <br /> * 3 copies of a site base map showing locations of borings <br /> (not drawn on back of permit application) <br /> * Authorization to release analytical results, geotechnical <br /> data, and site assessment information <br /> * Statement from drilling contractor authorizing Norcal to <br /> sign drilling permit application <br /> * $35 processing fee for drilling permit application <br /> As we discussed on the phone, I am hereby authorizing Health <br /> Department staff to bill Norcal for overtime hours worked at the <br /> rate of $52 . 50 per hour. The drilling is scheduled for the week <br /> of Feb. 5.
