10 12 4 Securing of Covers - All manhole and cleanout covers shall be removed and <br /> the openings maintained clear of any obstructions When hinged doors or lids are provided, <br /> they shall be secured so they cannot be closed (See Section 10 5 3 for required barricades) <br /> 10 12 5 Testing of Atmosphere - A qualified person shall make appropriate tests of <br /> the atmosphere in the confined space to ensure the following <br /> i Combustible gas and vapor concentrations do not exceed 10 percent of the lower <br /> explosive limit <br /> ■ Oxygen content is no less than 20 percent and no greater than 25 percent <br /> ■ Appropriate respiratory protective equipment and other appropriate personal <br /> protective devices have been provided for all employees when concentrations of <br /> toxic materials exceed established threshold limit values (TLVs) <br /> 10126 Continuous Monitoring - If the nature of the work to be performed <br /> introduces, or has the potential to introduce, harmful air contaminants, continuous <br /> monitoring of the atmosphere is required If tests indicate evidence of dangerous air <br /> contaminants, and/or the oxygen content drops below 20 percent, all personnel shall evacuate <br /> the confined space immediately <br /> 10 12 7 Ventilation - All confined spaces found to be unsafe must be ventilated by <br /> means of a positive mechanical exhaust system so arranged as to avoid recirculating <br /> contaminated air <br /> 10128 Buddy System - At least one standby person shall be stationed lust outside <br /> the access opening of any confined space while such space is occupied This person shall <br /> 0 Maintain continuous awareness of the activities and well-being of the worker(s) <br /> in the confined space <br /> 10-4 REV 4/92-EGI <br /> 111ra <br />
