GHD Job #: 03009-10016 <br /> 1868 East Hazelton Avenue <br /> Stockton, CA 95205-6232 <br /> Re: Permit Application for Monitoring Well Destructions <br /> Former Blue Star, 4040 East Main Street, Stockton, San Joaquin County, CA <br /> CVRWQCB Case #391140, San Joaquin County LOP Case#1350 <br /> Dear Mr. Yoakum: <br /> Please find enclosed two (2) executed permit application for the destruction of five (5) <br /> monitoring wells at 4040 East Main Street, Stockton, California. Three (3) monitoring wells are <br /> located on the subject site and two (2) monitoring wells are located directly across Oro Avenue <br /> on a separate parcel. This work is being performed under the directive of the Central Valley <br /> Regional Water Quality Control Board (CVRWQCB) and the Emergency, Abandonment, and <br /> Recalcitrant (EAR) Account. The CVRWQCB has granted site closure through the State Water <br /> resources Control Boards (SWRCB) Low-Threat Closure Policy (LTCP); therefore, to complete <br /> the closure process, the current monitoring wells must be properly destroyed. The CVRWQCB <br /> approval is included in the Workplan for Monitoring Well Destruction for the proposed work. The <br /> San Joaquin County Environmental Health Department has a copy of the Court Order Inspection <br /> Warrant that is Confidential and should be kept out of the public files. The proposed work is in <br /> response to the requirement for the LTCP. <br /> As discussed via telephone, the fee ($1,375.00) and the two permit applications are enclosed <br /> for your review and approval. It is also understood that an additional fee for inspector time on <br /> the site may be forthcoming.
