Box 388 <br /> Stockton, California 95201-0388 <br /> Application for Boring Permits <br /> Laymac Dehydrator Station <br /> Northwest Corner off Manthey Road and Henry Long Boulevard <br /> Lathrop, California <br /> Dear Ms. Lagorio: <br /> With this letter Harding Lawson Associates (HLA)submits a completed application for 4 soil borings at <br /> the Laymac Dehydrator Station(Location Map)in San Joaquin County. HLA is conducting Preliminary <br /> Environmental Assessment on behalf of Pacific Gas and Electric Company(PG&E) at approximately 70 <br /> PG&E Gas Dehydrator Stations in northern and central California. This work is being performed under the <br /> regulatory oversight of the Regional Water Quality Control Board- Central Valley Region(RWQCB). A <br /> copy of the RWQCB approval letter is attached. A description of the field procedures and methods is <br /> attached to facilitate your processing of the permits. <br /> Enclosed are a check for$323.00 covering the application fee and Site Mitigation Program fee, a <br /> completed permit application with an attached site plan, a Masterfile Record Information Form and <br /> General Program File Form, a California Licensed Contractor Questionnaire, a copy of the Well Drillers <br /> License with expiration date, a copy of the Workers Compensation certificate of coverage, and a copy of <br /> the well driller's authorization letter to sign permits. <br /> The field work is tentatively scheduled for May 14, 1997. The work will commence at about 8 AM and <br /> will probably take most of the two days to complete. Grout inspections will be scheduled at least 48 <br /> hours prior to the requested inspection date. Your ongoing cooperation on this project is greatly <br /> appreciated by HLA and PG&E.
