Annual monitoring system certification, leak detector testing, and spill container testing <br /> were last performed on April 24, 2013 and was completed today May 2, 2014, one calendar month past <br /> due. These tests are required once every 12 months, within the same calendar month. Take all <br /> necessary precautions to ensure testing is performed in a timely manner. Next testing is due by end of <br /> May 2015. <br /> 404. Vent transition sump was found without a water-tight lid. Secondary containment shall be <br /> constructed to prevent any water intrusion into the system by precipitation, infiltration, or surface runoff. <br /> Immediately install a compatible and approved lid for the vent transition sump. Submit a photographic <br /> evidence of the newly installed lid. <br /> 9999. HSC 25404(e)(4). Facility new operator must complete CERS submittals updating information on <br /> new ownership. Regulated businesses shall report program data electronically. The information shall <br /> include, but not limited to, hazardous material inventory, hazardous waste, underground storage tanks <br /> and emergency response and training plans.
