Financial responsibility documents have not been submitted to the EHD. Current financial <br /> responsibility documents are required to be submitted annually. Complete and submit a copy of the <br /> financial responsibility by 05/26/2012. <br /> 205. The November 2011 designated operator monthly inspection report was not found on site. <br /> Designated operator monthly inspection reports for the previous twelve months shall be retained on site. <br /> Locate and ensure that copies of the previous twelve months of designated operator monthly inspection <br /> reports are maintained on site. Submit a copy of the November 2011 designated operator monthly <br /> inspection report to the EHD by 05/26/2012. <br /> 310. Approximately 1 quart of liquid was observed in each 87-octane fill sump, 91-octane STP and fill <br /> sumps, and diesel fill sump. If water could enter into the secondary containment by precipitation or <br /> infiltration, it must be removed and disposed of properly. Immediately remove this liquid, make a <br /> hazardous waste determination per Title 22 hazardous waste regulations, and manage it accordingly. <br /> Ensure that the sumps are maintained free of liquid. Technicians removed the liquid and placed it into <br /> hazardous waste container on site; violation was corrected on site. <br /> 311. The 91-octane and diesel grade line leak detectors failed to detect whe ed. All line leak <br /> detectors shall be capable of detecting a 3-gallon per hour lea psi. ediately have a properly <br /> licensed, trained, and certified contractor repair or re a fail a detector (LG 113-x listed, if <br /> applicable) and obtain a permit within one ss day frowrtfie EHD. If the failed leak detectors are <br /> not repaired or replac�youretest the er purgingair from fuel lines as discussed on site, <br /> please be aware thatDquired and the time is billable at current EHD hourly rate. <br /> If the failed leak ctors can't be ierritced or retested immediately, there is a possibilitythat the UST <br /> system a red to a revent fuel inputs.
