A disposal well 48 inches in diameter and 25 feet deep was installed on this parcel in
June 1971 to receive waste water from process washing of fiber glass molds made by American
Standard. American Standard moved its operations to another site and the disposal well was
never properly destroyed.
EHD has implemented a Local Agency Groundwater Protection/Underground Injection Control
Program in accordance with guidelines prepared by the United States Environmental Protection
Agency(USEPA). The primary focus of the program is the identification, evaluation, and potential
closure of improper disposal wells and identification of other land use practices in San Joaquin
County that may lead to degradation of drinking water supplies
This site is currently classified as an Underground Injection Control Program Environmental
Assessment, assigned to the Site Mitigation Unit at EHD. EHD requests you read, complete,
sign, and return the enclosed Master File Record Form and submit the required 3 hour minimum
advanced fee for services totaling $267.00 (Payment to: EHD).
EHD requests that you submit the forms, fees and a proposal for destruction of the
disposal well and for environmental sampling that will confirm that the subsurface has not
been contaminated by use of the disposal well by March 1,2002.
Should you have questions regarding this correspondence please contact me at (209) 468-3449.
Donna Heran, REHS, Director
Environmental Health Division
"
_A
Margaret Lagorio, Supervising REHS
Site Mitigation Unit
Enclosures—Master File Record Form
A Division of San Joaquin County Health Care Services
