Ms. Moore, the addressee of <br /> your letter, is currently on maternity leave and this issue has been referred to me. <br /> I retrieved the referenced file to look into the matter. In the file I did find a fax from you <br /> containing the required application and information that needed to be submitted to PHS/EHD for <br /> the drilling. The application contained in the file was filled out by Ms. Moore and a Scope of <br /> Work document prepared. However, I cannot find records of these documents being forwarded <br /> to PHS/EHD or a record of the permit application fee being paid.
