Health and Safety Plan 1 <br /> Sears,5110 Pacific Avenue, Stockton, California October 22, 1999 <br />' 1 0 INTRODUCTION <br />' Sears, Roebuck & Co has retained IT Corporation, Inc to perform environmental services at 5110 <br /> ' Pacific Avenue, Stockton, California <br /> This Health and Safety Plan (HASP) is written to ensure the well-being of all field personnel and <br /> ' the community surrounding the site Accordingly, project staff and approved IT Corporation <br /> subcontractors must follow the policies and procedures established in the HASP All personnel <br /> assigned to this project must sign the Agreement and Acknowledgment Sheet (Appendix A) to <br /> ' confirm that they understand and agree to abide by the provisions of this plan <br /> All work will comply with the CAL/OSHA standard, "Hazardous Waste Operations and Emergency <br /> Response," Title 8, California Code of Regulations (CCR), section 5192, and other federal, state <br /> and local procedures that require the development and implementation of a HASP Generation of <br /> this document certifies that the workplace has been evaluated for the hazards as described A <br /> ' hazard assessment has been performed and the adequacy of the personal protective equipment <br /> (PPE) selected is hereby certified per Title 8 CCR, sections 3380 - 3387 and is duly noted by the <br /> signature(s) and date appearing on the cover page of this document <br /> This plan addresses the safety issues associated with retail petroleum station operations typically <br /> ' involving the following site tasks <br /> Field Survey/Walkover ■ Aquifer Pump Test <br /> ' ■ Drilling/Boring/Soil Sampling ■ Tank Removal <br /> ■ Well Installation ■ Excavation/Trenching <br /> B Well Monitoring and Maintenance ■ Confined Space Entry (CSE) <br /> ' ■ Soil Gas Survey ■ Groundwater Sampling <br /> All activities of this project will be carried out under Level D, Modified Level D, or Level C PPE <br /> ' This HASP must be modified or amended when circumstances or conditions develop that are <br /> beyond the scope of routine gas station operations Such conditions include <br /> 0-csonce of (`hinrinated Snlventc ■ Troubleshootinq Energized <br /> s Presence of Pesticides Equipment <br /> ■ Air Stripper Cleaning ■ Presence of Nonhydrocarbon <br /> ■ Level B Work Materials <br /> Any changes in project work scope and/or site conditions as described must be amended in writing <br /> • <br />
