BRADSHAW LEVINE-FRICKE-RECON INC. <br /> 1900 POWELL ST. 12 TH FLOOR <br /> EMERYVILLE CA 94608-1827_ <br /> In accordance with Ordinance 003-96, passed by the Council of the City of Stockton on January <br /> 22, 1996, a permit revocable at the pleasure of the Public Works Director of the City of Stockton <br /> is hereby granted to: <br /> SETT LING DRY CLEANING DEFENDANTS <br /> to install and maintain three (3) monitoring wells in Pacific Avenue East Frontage Road,150' North <br /> of Benjamin Holt Drive, Stockton, CA. <br /> This permit is granted under the following terms and conditions: <br /> 1. This revocable permit is granted subject to the Standard Conditions Applicable to <br /> Revocable Permits adopted by this City Council by Resolution No. 22,578 on December 5, <br /> 1960, and on file in the Office of the City Clerk of the City of Stockton. <br /> 2. The general location of each well shall be as shown on the map attached. The precise <br /> location shall be determined in the field by the City of Stockton. <br /> 3. The Permittee has posted security of $1,500.00 with the City of Stockton which will remain <br /> in effect during the life of this permit. <br /> 4. In accordance with Section 1(h) of the "Standard Conditions Applicable to Revocable <br /> Permits,' the permittee shall provide and keep in force during the term of the permit a policy <br /> of public liability insurance against liability for injuries to persons or property arising out of <br /> accidents attributable to the structure on City property, in amounts to be determined by the <br /> Risk Manager. The policy of insurance so provided shall contain a contractual liability <br /> endorsement covering the liability assumed by the Permittee by the terms of his permit and <br /> shall contain a provision that such policy may not be canceled except after thirty (30) days <br /> notice in writing to be given to the Risk Manager. The copies of these policies or <br /> certificates evidencing the same shall be filed with the Risk Manager of the City after <br /> approval of the City Attorney. <br />
