Levine•Fricke•Recon
ENGINEERS,HVDROGEOLOGISTS 8 APPLIED SCIENTISTS
Field activities will be conducted generally between the hours of 8:00 p.m. and 7:00 a.m. on
Sunday through Thursday evenings. We estimate that approximately three night shifts will be
needed to install each extraction well and two night shifts will be needed to install each
piezometer.
Before LFR can install the extraction wells on LPL property, we must obtain well permits from
the San Joaquin County Public Health Services, Environmental Health Division (SIC PHS/EHD).
To issue the permits, the SIC PHS/EHD must have written permission from the property owner to
conduct the work. Therefore, please sign the acknowledgment in the space indicated, transmit the
signed document to me via facsimile at (510) 652-4906, and return the original document to me in
the enclosed self-addressed, postage-paid envelope.
If you have any questions or comments regarding our request, please do not hesitate to call me at
(510) 596-9559. On behalf of the SDCDs, we thank you for your cooperation in this matter.
Sincerely,
Donald T.
