LFR
I •FRICKE
January 10, 2006 0 50-05
JAN 11 2006
ENVIRONMENT HEALTH
Mr. Mike Infurna PERMIPSERVICES
San Joaquin County Environmental Health Department
304 East Weber Avenue, 3`d Floor
Stockton, California 95202
Subject: Well/Soil Boring Permit Application
Lincoln Center, Stockton, California
Dear Mr. Infurna:
Enclosed please find a well permit application for the Lincoln Center site located in Stockton
California ("the Site"). As discussed in our telephone conversation, LFR Inc is submitting these
permit applications to supplement the November 9, 2005 well permit previously submitted. The
contractor for the work has been switched to Precision Sampling Inc (Original permit was for
Vironix). In addition, further research revealed that the work will take place over two separate
parcels, therefore requiring two separate permit applications.
LFR will advance up to 14 soil borings (MIP-1, MIP-2, MIP-3, MIP-4, MIP-5, MIP-6, MIP-7,
MIP-8, MIP-9, MIP-10, MIP-11, MIP-12, MIP-13, and MIP-14) within the Lincoln Center
parking lots located no and south of Benjamin Holt Drive (Figure 1). The work is scheduled to
take place from'Monday January 16, 2006 through Friday January 20, 2006. The soil borings will
utilize membrane interphase probe (MIP) technology, and will be advanced to approximately 75
feet below ground surface (bgs) with a direct push drill rig. This project is being completed with
the oversight of the Regional Water Quality Control Board (RWQCB). To process the enclosed
well permit application, the following items are enclosed:
• Two well permit applications for the parking lots located north and south of Benjamin Holt
Drive;
• Check Numbers 121876 and 121881, made payable to the San Joaquin County of
Environmental Health Department in the amount of$89 each. These checks have been
provided to satisfy the well permit application fees.
• Check number 121879, made payable to the San Joaquin County of Environmental Health
Department in the amount of$465.00. This check is provided to satisfy a past due amount
resulting from previous oversight activities conducted by San Joaquin County.
• Copy of previously approved well permit application pp !FS -
061 f yQ C%
1900 Powell Street, 12 Floor, Emeryville, California 94608-1827 (510)652-4500 fax(510)652-2246 www.Ifr.com
Offices Nationwide
