ARCADIS <br /> Monitoring Well MW-3 <br /> MW-3 is a 4-inch OD PVC groundwater monitoring well and was installed by Sierra on July 25 and 27, <br /> 1990. The monitoring well log provided by Sierra (Attachment 1)indicates that the total depth of the <br /> monitoring well when installed was 68 feet bgs with a screen interval of 48 to 68 feet bgs.As detailed on <br /> the monitoring well log the borehole was drilled to a total depth of 71.5 feet bgs, however,the total depth <br /> of the monitoring well was 68 feet bgs. A note is not provided on the boring log explaining the method <br /> used to backfill the interval from 68 feet bgs to 71.5 feet bgs; however, it was likely a bentonite grout <br /> mixture. <br /> Inspection Findings <br /> Monitoring well MW-3 is currently unsecured. Although the monitoring well cover was in place,the bolts <br /> appear to be the incorrect length resulting in the cover not being firmly secured to the well box.
