Surban did exactly what I told him to do and a good job at
it too..Unfortunately, at the time I didn't open ALL of the well heads but told him to repairl all the wells. HE
did...but some were NOT wells..
There are some few things needing your attention before I sign this off...
please have your people address/repair the following items. When they tell you they are completed, get in
touch with me and I'll schedule a re-inspection.
1- MW-12 needs grout INSIDE traffic box lowered. Grout is AT the top of the PVC casing and they are both
are too high in the traffic box (touching the bottom of the lid) and not allowing proper locking/securing of the
well head.
2- there's a -14' deep by 10" diameter steel casing inside a traffic box located inbetween the center and
east UST. This 10"-steel casing APPEARS to be a 'conductor casing' placed inbetween the USTs at UST
installation time for future confirmation soil sampling/drilling AFTER the site completes its remediation and
qualifies for No Further Action. My notes indicate that I advised these could be installed so drilling
BETWEEN USTs later would not be so dangerous. Unfortunately, this casing is FULL of water with a slight
sheen (petro?)on the surface. Allowing liquid (contamination) to migrate from the surface to 14' bgs is
unacceptable. This site does NOT need further contamination/sources. I'm confident there is a cement
grout bottom (at 14' bgs) preventing MOST of the liquid migrating into the soil, but I'm not sure how
impenetrable it is. What I want is this well head/lid to be WATERTIGHT...I will accept a silicone seal being
applied to the inside of the lid, AFTER ALL the water is pumped out and containerized. But if you have
other thoughts on how to seal it, fine...What I want is NO water to enter this casing, as the final result.
3-1 recommended that this 'conductor casing' box, as well as a 'sewer' cleanout box (that has a Monitoring
well box and lid) be sealed with unique bolt/screws so that normal monitoring and sampling techs DON'T
have the correct tool to open there—just a suggestion since these boxes appear to be wells, but aren't
really...
let me know.
-----Original Message-----
From: Network Administrator [PHS]
3/22/2006
