Well Installation and Repair Work Plan Letter Addendum April 13,2011 <br /> Page 2 <br /> necessary well installation and encroachment permits, ATC will work to commence with field work on Monday, <br /> May 16, 2011. Once the proposed wells are installed and the protected wells are recovered, Miller Pacific Inc. will <br /> pave the site and set the necessary well vaults flush with the surrounding pavement. Once the well vaults are set, <br /> ATC will have the wells surveyed, developed, and sampled. Well installation and recovery activities will be <br /> documented in a report to be prepared by ATC. If delays in performing the proposed well installation and well <br /> recovery activities are encountered, ATC will notify the San Joaquin County Environmental Health Department of <br /> those delays. <br /> Limitations <br /> This work plan letter addendum was prepared in accordance with the scope of work outlined in ATC's contract. It <br /> was prepared for the exclusive use of ConocoPhillips, for the express purpose stated above. Any re-use of this <br /> work plan for a different purpose or by others not identified above shall be at the user's sole risk without liability to <br /> ATC. To the extent that this work plan is based on information provided to ATC by third parties, ,ATC may have <br /> made efforts to verify this third party information, but ATC cannot guarantee the completeness or accuracy of this <br /> information.
