As soon <br /> as the correct check is issued (for the amount of $500), ATC will submit a second application packet for the <br /> shallow soil vapor sampling activities. <br /> Items included in this permit application packet are: <br /> • One (1) San Joaquin County EHD Well Permit Application. <br /> • One (1) completed Licensed Contractor Declaration Form. <br /> • One (1) completed Master File Record (MFR) Form. <br /> • A site plan showing the locations of the proposed assessment locations. <br /> • A check in the amount of $1,250.00, payable to San Joaquin County, for the installation of six on-site <br /> monitoring wells and the advancement of one soil boring. <br /> Any completed well permits can be sent to my attention at 25 Cupania Circle, Monterey Park, CA 91775, or <br /> electronically to ben.chevlen(_)atcassociates.com.
