6.0 DOCUMENTATION <br /> The field documentation required of the Field Supervisor will include <br /> recording all observations made during drilling, sampling, or decon- <br /> tamination that could affect the quality of a sample. The documenta- <br /> tion for each sample should be entered in a bound field notebook with <br /> consecutively numbered pages and/or the data collection forms. <br /> Documentation should include at a minimum: <br /> • Date of drilling/location/diagram or map. <br /> • Date of sample collection. <br /> • Time of day. <br /> • Borehole ID number. <br /> • Depth of sample. <br /> • Type of sample (e.g., soil or water, QA/QC sample). <br /> • Sampling method (e.g., California-modified sampler or Continu- <br /> ous sampler). <br /> • Boring method (e.g., 8-inch hollow-stem auger). <br /> • Surrounding conditions (e.g., weather, etc.). <br /> • Unusual conditions (e.g., volume of water removed prior to sam- <br /> ple collection). <br /> • True vertical depths. <br /> • Decontamination procedures. <br /> • FID or PID readings. <br /> • Generalized boring log with sample locations, sample numbers, <br /> and general lithology. <br /> • A brief description of the area around the boring/monitoring well <br /> location and the weather conditions at the time of sample collec- <br /> tion. Each entry (or page) in the field notebook should be dated <br /> and initialed by the individual making the entry. <br /> Detailed field borehole logs shall be completed as described in Section 4 <br /> and kept by the Field Supervisor. <br /> 6-1 <br />
