6. Monitoring Well Installation and Destruction Report <br /> City of Stockton RWCF <br /> Page 6 <br /> 4.0 MONITORING WELL MW-4 DESTRUCTION <br /> Condor prepared the Monitoring Well Destruction Work Plan, dated August 4, 2009. Work plan approval <br /> L and authorization to proceed was provided in a letter dated August 17, 2009 to Mr. Stephen Gittings <br /> (MUD) from Ms. Gayleen Perreira of the Regional Water Board. The well was properly destroyed in <br /> accordance with San Joaquin County guidelines and State of California Department of Water Resources <br /> LBulletin 74-81 and 74-90 regulations. The well destruction was observed by Mr. Frank Girardi of <br /> SJCEHD. <br /> '` A track mounted limited access drilling rig with 10-inch diameter augers was used for the well <br /> destruction. The groundwater monitoring well was destroyed by placing a PVC cutting tool (casing <br /> _ reamer) into the well, then over-drilling using hollow-stem augers to a depth of 15 feet. The casing <br /> reamer served the dual purpose of grinding down the casing and guiding the augers to keep them centered <br /> V on the casing. Pieces of broken well screen were observed in soil cuttings brought to the surface. Cement <br /> grout was injected into the boring through tremmie pipe from the bottom upward. Native soil and sand <br /> were used to fill the remaining boring to the ground surface. <br /> L <br /> 5.0 LIMITATIONS AND SIGNATURE <br /> LCondor has endeavored to determine as much as practical about the Site using conventional practices <br /> given our scope of services. Conclusions presented in this report are professional opinions based on <br /> limited information obtained at the time of our Site visits. Condor makes no representation as field <br /> L <br /> conditions at locations or times other than those reported in this document. If any changes are made or <br /> L errors found in the information used for this report, the interpretations and conclusions contained herein <br /> shall not be considered valid unless the changes or errors are reviewed by Condor and either appropriately <br /> Lmodified or re-approved in writing. <br /> Condor's responsibilities discussed in this report are limited to observing well installation and destruction, <br /> collecting and recording depth-to-water measurements and field parameters, and interpreting field data. <br /> L Condor is not responsible for the accuracy and completeness of information collected and developed by <br /> others. This report was prepared by Condor under the direct supervision of a Certified Engineering <br /> Geologist in the State of California.
