Sorta late, but better than nothing. Yes, there was and still are some missing papers for this phase. Enclosed is the permit for onsite "OW-1" destruction. I CANNOT issue for the offsite well's installation without: (1) copies of the right of way `encroachment' permits from the correct municipal entity(city or county public works dept), AND 2) $89.00 check. It is premature for you to schedule offsite monitoring well installations WITHOUT the PHS-EHD permit issued. You may start the ONSITE work only that relates to this permit copy I'm sending here. Without me verifying that the EHD permit is issued, you may NOT start the offsite MW installations...OK? . Call me and verify your intentions.
