The following is an itemized list of hazardous materials violations that have not been <br />addressed for TIGER EXPRESS STORES as of April 01, 2020. <br />Open violations from September 07, 2018 inspection <br />�,� Violation #5 - Failed to complete and/or submit hazardous materials inventory for all reportable materials on <br />site. <br />300 pounds of carbon dioxide , 73.5 gallons of propane were found on site that have not been reported. Any <br />material that meets or exceeds the reportable quantity shall be reported in the facility's business plan. The site map is missing loading areas for the carbon <br />dioxide and the fuel, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency <br />shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency response <br />equipment fuel fill port locations . If a site map element is not applicable for your facility then list it on the map and <br />label as "NA". Please fill out section C2, C3, F, and fill out the locations in <br />section G of the emergency response and contingency plan. The local Unified Program Agency (UPA) phone <br />number for section C3 is 209-468-3420. <br />A business plan shall include the following emergency response procedures for a release or threatened release of <br />hazardous materials, including, but not limited to, the following: <br />- immediate notification of local emergency personnel and the Environmental Health Department (EHD) <br />- procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br />persons, property, or the environment <br />- evacuation plans and procedures, including immediate notice, for the site <br />Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br />correct or updated information, and submit to the EHD for approval. <br />Page 1 of 3 <br />
