This $3/hour increase modified the 3-hour prepayment from $285 to $294.
This new fee, plus the existing $89 permit fee now equals $383, not the$374 you sent in.
Please provide the$9 balance to the EHD as soon as you can. I cannot work on the work plan
(review)or permit issuance until the proper fee is received.
As you mentioned on the phone today, payment at the counter of our newly relocated office is
acceptable. Just note on the fee, "Attn Mike Infurna".
In case you are unaware, our new office location is now 600 E. Main Street, Stockton, on the corner
of American Street and E. Main Street. We are easily accessed by the crosstown freeway, just off
the Stanislaus Street exit. Two blocks north on Stanislaus St and left (west)on Main Street. Our
office is on the left side, one block down Main at the comer of American St. Counter hours are 8am
to 5p...and we are open during the lunch hour.
Below for your future referrence is our webpage detailing how to obtain a boring permit in the
contamination investigation unit(Unit IV)
http://wwwsjgov.org/ehd/docs/pdf/Requirements%20for%2OUn it%201V%2OWell%2OPermitl.pdf
Mike Infurna, REHS
SJCEHD
(209)468-3454
9/27/2007
