.Daily Tailgate Meeting <br /> Site: Start Date: <br /> Completion Date: <br /> ❑ Site Plan with JSAs and MSDSs <br /> ❑ Possible hazards and known contaminants on-site <br /> ❑ PPE required (Hard hat, Gloves, Safety Glasses, Coveralls, Steel Toed <br /> Boots) <br /> ❑ Identify and communicate emergency routes <br /> ❑ Locate the nearest hospital. <br /> ❑ Locate the fire extinguisher <br /> ❑ Locate the emergency shut-off switch (at active service stations) <br /> ❑ Everyone signs the Site Safety Plan <br /> Scope of Work <br /> ❑ Number of wells/borings—(clear to 8 feet) <br /> ❑ Sampling interval <br /> ❑ Identify any traffic and/or pedestrian hazards <br /> ❑ Identify the site constraints (i.e any difficulties that may be <br /> encountered during drilling; near dispensers; lithology) <br /> ❑ Identify overhead and subsurface hazards, <br /> ❑ Establish safety perimeter around equipment and work zone <br /> ❑ Proper lifting of equipment used to transport heavy objects (i.e. drums, <br /> augers) <br /> ❑ Stockpile and/or drum location <br /> ❑ Check kill switch on rigs and inspect all equipment <br /> Communicate time constraints (i.e. if you must be off street/sidewalk <br /> at certain time) <br /> ❑ Communicate time of grout inspection. <br /> ❑ Ensure there are observation cards (LPO, SPSA) available/on-site <br /> No dirt or slurry is allowed in storm drains <br /> ❑ Have Stop Work Authority <br /> ❑ Leave the site clean! <br /> Comments and Site Specific Issues: <br />
