RADIAN INTERNATION-kL <br /> A DAMES&MOORE GROUP CComp" <br /> l �Ill <br /> To: Karen Bessette (RWQCB) MEMORAN� <br /> Harlan Knoll (San Joaquin County EHD) <br /> From: Dick Van Dyke (Radian International) <br /> Copy: Major David Sheets (CEHNC-PM-ED) <br /> Mike Williams (CEHNC-ED-CS) <br /> Marshall Cloud (DDJC) <br /> Ron Allen (DDJC-PE) <br /> Graham Sharpe (Radian International) <br /> Project File (602227) <br /> Date: 20 October 1999 <br /> Subject: Submission of SHAD Well 2 Video Survey, DDJC-Sharpe (DACA87-95-D- <br /> 0001/ 0027) <br /> The purpose of this memorandum is to present one copy of the down-hole video survey recording, <br /> which documents the condition of the inside of the casing of SHAD Well 2 at DDJC-Sharpe.This <br /> video was recorded on 15 October 1999 by our subcontractor,Layne Christiansen Co.The video <br /> recording was produced in response to a request by the San Joaquin County Environmental Health <br /> Department and the California Regional Water Quality Control Board for a down-hole video survey <br /> to be completed prior to approval of the proposed well abandonment method of pressure grouting. <br /> This work was conducted as part of the effort to abandon SHAD Well 2, as documented in the <br /> attached work plan (dated 9 September 1999). <br /> Based on review of the video survey, the following observations are made: <br /> • The well total depth is 290 feet below ground surface (bgs); <br /> • The well bottom is filled with sediment (well data record indicates a well depth of 333 feet bgs); <br /> • The perforated intervals of the well casing are 122-126, 176-182, and 192-206 feet bgs; and <br /> • The steel casing appears to be competent, with no observed holes,cracks, or offset joints. <br /> The proposed well abandonment method is by pressure grouting through a tremie pipe, as described <br /> in the attached work plan.Well abandonment will not proceed until notification is received from the <br /> County/RWQCB approving the abandonment method.
