Box 2009 <br /> Stockton,CA 95201 <br /> Dear Ms.Ligorio: <br /> Enclosed are the completed Permit Applications and Request for Services forms for the <br /> upcoming soil investigation at DDRW- Sharpe in San Joaquin County. I have also enclosed <br /> check number 135761 for$440,which covers the$89 application fee, three hours of review time, <br /> and a$117 rush fee. <br /> The activities planned for the soil investigation at DDRW - Sharpe include drilling 70 soil <br /> borings and installing two monitoring wells at two sites on the depot property. The soil borings <br /> will be drilled to 15 feet below ground surface. The borings will be abandoned with neat cement <br /> grout tremied from the bottom of the hole. The monitoring wells will be installed to 8 feet below <br /> fust water. Once completed, a copy of the Water Well Driller's Log will be submitted to the State <br /> of California and SJCPHS. <br /> The shallow monitoring wells and the soil borings will be installed using hollow-stem auger <br /> drilling rigs supplied by West Hazmat Drilling of Newark, California. Copies of the Work Plan <br /> for the investigation are contained in the enclosed binder. <br /> I understand that a representative of SJCPHS Environmental Health Division will need to observe <br /> the backfilling of the boreholes and installation of sanitary seals within the monitoring wells. We <br /> will make every effort to provide you with as much notice as possible regarding our grouting <br /> schedule.
