Use of the Manifest <br /> Hazardous wastes shipped off site to treatment, storage, disposal or <br /> transfer facilities must be accompanied by an accurately completed hazardous <br /> waste manifest form. Procedures followed at Sharpe relating to manifest <br /> forms and sampling and testing of hazardous wastes are described below. <br /> 1. Manifest Preparation Instructions and Operational Procedures <br /> a. Instructions for completion of Uniform Hazardous Waste Manifest <br /> DHS 8022A: <br /> (1) Item 1 . Generator U.S. EPA ID NUmber - Manifest Document <br /> Number: Enter the generator' s U.S. EPA twelve-digit identification number <br /> and in the space to the right of this line enter a five-digit number of your <br /> choice. <br /> • (2) Item 2. Page 1 of Enter the total number of pages used <br /> to complete this manifest plus the number of Continuation Sheets, if any. <br /> (3) Item 3. Generator' s Name and Mailing Address: Enter the <br /> name and mailing address of the generator.
