Box 400, <br /> Sacramento, CA 95812-0400 <br /> Yellow: Generator retains and places a 30-day suspense for receipt of <br /> second-yellow copy from TSDF. , <br /> (5) Other Transporters will sign and date the manifest <br /> acknowledging the receipt of materials from previous transporter. <br /> (6) (TSDF) Facility Owner or Operator will sign and date the <br /> manifest certifying receipt of hazardous materials. Any discrepancies in the <br /> manifest are noted on block 19 of the manifest. <br /> (7) TDSF will distribute signed manifest as follows: <br /> GREEN: Hauler Retains <br /> WHITE: TSDF Retains <br /> WHITE: TSDF sends this copy to DOHS within 30 days to: <br /> P.O. Box 3000, Sacramento, CA 95812 <br /> YELLOW: TSDF sends this copy to generator within 30 days <br /> (8) Upon receipt of yellow copy from TSDF, the Generator shall <br /> clear suspense., If copy of manifest is not received by Generator from RSDF <br /> • within 30 days,' notification will be made in writing to the California <br /> Department of Health Services. <br /> (9) Copy of each manifest and associated documents will be <br /> retained for three years.
