This HSP has also been designed to meet guidelines for the Comprehensive Long-Term <br /> Environmental Action Navy (CLEAN) Health and Safety Program dated April S, 1991, and to <br /> comply with the U S Navy's environmental health and safety requirements <br /> ' All participants in the sampling activities will be familiar with this site-specific HSP and will <br /> sign the acceptance form (Attachment A) A feedback form is provided for constructive comments <br /> ' regarding the HSP (Attachment B) <br /> ' Technical and construction subcontractors are obligated to conform with standard industry <br /> safety practices for their profession including OSHA compliance If the subcontractor proposes <br /> ' changes in the HSP, the On-Site Health and Safety Coordinator (OHSC) shall obtain permission from <br /> the Health and Safety Program Manager (HSPM) and Project Manager (PM), and this authorization <br /> will be documented in the project site logbook A modification to the HSP reflecting the changes will <br /> be issued to the subcontractor <br /> Technical and construction subcontractors must certify, by name, that each of their own <br /> employees who will perform field work at a hazardous waste project site has been medically certified <br /> by a physician for respirator usage within the past 12 or 24 months, in accordance with 29 CFR <br /> ' 1910 120 (f)(3), prior to the work being performed In addition, the subcontractor must certify, by <br /> name, that each employee who will perform field work has received health and safety training, <br /> including the selection, use, and care of a respirator <br /> The OHSC will be responsible for ensuring that all personnel entering an active work area <br /> ' comply with the HSP, meet the medical and training requirements for the site, and have the required <br /> level of personal protective equipment (PPE) Only authorized personnel will be allowed in active <br /> ' work areas Daily site safety meetings will be held each morning before work begins and workers <br /> will acknowledge their attendance by signing the safety meeting sign-off sheet (Attachment C) <br /> ' Accidents or incidents at the job site that affect or could potentially affect worker safety will be <br /> documented using the accident/incident report form (Attachment D) <br /> 1 <br /> The OHSC will establish the initial level of PPE and respiratory protection and will have the <br /> ' authority to upgrade or downgrade levels of protection at any time in response to field conditions <br /> Cf0162 NSP 2 <br /> 044-0162UPATR!-05,%M <br />
