' 6.0 STANDARD OPERATING GUIDELINES <br /> Prior to initiation of any on-site field activities, the OHSC will notify the nearest hospital of <br /> the planned site activities and potential medical needs The OHSC will also conduct a safety meeting <br /> ' to familiarize personnel with the route to the hospital and discuss the removal and sampling <br /> procedures and any high-risk activities In addition, the OHSC will contact local fire protection and <br /> ' police organizations prior to start of activities to apprise them of planned operations and the nature of <br /> possible emergencies <br /> 1 <br /> The site perimeter will be identified with temporary barriers and plastic caution tape A <br /> ' minimum of 50 feet of clearance will be maintained around drilling activities <br /> The OHSC has the authority to upgrade or downgrade levels of protection at any time in <br /> response to field conditions, and may restrict access to the vicinity of field activities, as deemed <br /> ' necessary, regardless of the protective gear worn Personnel not immediately involved with field <br /> work should remain outside of the exclusion zone Contact lenses may not be worn during the field <br /> work All personnel should provide their own prescription safety glasses If necessary, PRC <br /> employees will be provided with prescription safety glasses for use with the full face respirator <br /> ' Field personnel should familiarize themselves with the Emergency Response Plan detailed in <br /> Attachment M In the event that an accident occurs that requires emergency care, field personnel will <br /> ' render appropriate first aid The OHSC will notify the necessary agencies and individuals An <br /> accident/incident report form will be completed by the OHSC (Attachment D) <br /> 6.1 INITIAL LEVEL OF PERSONAL PROTECTION <br /> O Level A O Level B { ) Level C (X) Level D <br /> 1 <br /> The field activities will be performed in Level D PPE Sampling activities will be conducted <br /> with continuous air monitoring The OHSC will have the option of upgrading to Level C PPE or <br /> higher if airborne contaminant levels are determined to be greater than previously monitored <br /> ' background levels Air monitoring action levels are discussed in Attachment N <br /> cr0162 HSP 14 <br /> 04"i62UTATR1-W/W193 <br />
