A well permit is required for subsurface boring,monitoring or other purposes. <br /> PHS-EI-ED well permits are issued to properly licensed contractors for each site (parcel) and are <br /> valid for: <br /> • only the contractor noted on the permit, <br /> • only for the work described in the work plan or as noted by the staff member comments on the <br /> permit, <br /> • one year or until they have been finalized by the PHS-EHD inspector. <br /> In order to evaluate if a permit should be issued for sub-surface boring investigations that include <br /> sampling for contaminants, a work plan, accounting forms and associated fees must be submitted. <br /> Once the permit is issued, appointments with the PHS-EHD staff in the Site Mitigation Unit (IV) <br /> must be scheduled before the work is performed. <br /> our records indicate that no permit was issued for either contaminated or non-contaminated <br /> subsurface investigations at these two sites for the dates noted above. Unless you can provide <br /> copies of well permits issued for the work done on the dates noted above, permit applications, <br /> permit fees and penalties are now due. <br /> In conversations with Mr. Gary Pischke of ESCNC,PHS-EHD has learned that "perc-tests" were <br /> performed at 1881 Rustan Street by Spectrum Exploration on March 24, 1999 under a PHS-EHD <br /> well permit (SR#018688) that was finalized on April 2, 1999. It appears that ESCNC may have <br /> incorrectly advised Gregg Drilling and Exploration Geoservices that the permit issued to Spectrum <br /> Exploration was valid for the work they had contracted with you. <br /> A Division of San Joaquin County Health Care Services <br />
