Mr.Harlin Knoll <br /> Checklist for No Further Action Request and Monitoring Well Destruction Work Plan <br /> September 29,2010 <br /> Page 5 <br /> • Monitoring wells MW-2 and MW-4 will be overdrilled and removed using an auger <br /> drill rig. This will include the removal of the PVC piping,bentonite,sand,grout, <br /> and well box. The concrete and metal will be placed in the regular trash;the <br /> remaining material will be spread on site in an area of minimal erosion activity. A <br /> Portland cement/bentonite grout will be used as backfill material. The selected <br /> licensed driller will follow the California Well Standards,Bulletin 74-90 (Supplement <br /> to Bulletin 74-81). <br /> • All decommissioning activities will be documented by SHN field personnel. <br /> • A report documenting site activities will be submitted to the SJCEHD within 90 days <br /> following the conclusion of fieldwork activities. <br /> Proposed Schedule <br /> SHN is prepared to commence work on the destruction of the monitoring wells within four weeks <br /> after a NFA letter and well destruction permits have been granted by the SJCEHD.
