Analytical results of the soil samples <br /> taken from the excavations indicated the presence of underground petro1eum <br /> contamination at both former 20,000 gallon USTs and the former 2,000 gallon <br /> UST. <br /> In a "Notice of Responsibility" addressed to Mr. Chris Walker dated April 25, <br /> 1997, San Joaquin County Public Health Services, Environmental Health <br /> Division (PHS/EHD) officially notified you that your site has been assigned to the <br /> PHS/EHD Local Oversight Program for oversight of assessment and clean up <br /> activities at contaminated underground storage tank sites. Site closure cannot <br /> be considered until PHS/EHD has determined that no further site investigation or <br /> remedial action is needed <br /> In a telephone conversation on July 19, 1996, Mr. Chris Walker requested an <br /> application to the State Water Resources Control Board (SWRCS) Cleanup <br /> Fund. After submitting the application in 1997, Mr. Chris Walker was informed in <br /> a letter dated February 23, 1998 from the SWRCB UST Cleanup Fund that <br /> additional documentation was needed in order to determine eligibility for <br /> placement on the Cleanup Fund Priority List . When the requested <br /> documentation was not provided, you were notified of the ineligibility of your <br /> claim application in a letter dated April 13, 1998 from the SWRCB UST Cleanup <br /> Fund. <br /> Enclosed is a SWRCB Cleanup Fund application, copies of the two letters from <br /> the SWRCB Cleanup Fund referenced above, and a list of environmental <br /> consulting firms. <br /> A Division or San Joaquin county Heaith c <br /> .M Sa.icet <br />
