Present at the meeting were you,Dan Barber representing D.K.Barber P.E. <br /> &Associates,and Jeffrey Wong representing PHS/EHD. <br /> Five Geoprobe borings were advanced and monitoring well#1 (MW-1)was installed between <br /> November 2000 and January 2001 at the site. PHS/EHD has received the analytical results of the <br /> collected soil and groundwater samples by facsimile;however,PHS/EHD has not received the <br /> complete report of the investigation work to date. <br /> At the above-mentioned meeting,PHS/EHD agreed that the investigation report and the upcoming <br /> fust quarterly monitoring report maybe combined into a single report. The deadline for submission <br /> of this report is May 1,2001. <br /> PHS/EHD expressed at the meeting that based on the analytical results,additional work to <br /> investigate the extent of the contamination in groundwater may be warranted. However,the final <br /> decision for additional work requirement will be made after PHS/EHD has received and reviewed <br /> the report. <br /> A 2,000-foot well survey is required for underground storage tank site closure according to the <br /> Appendix B of the Tri-Regional guideline.
