The letter stated that submittal of a
work plan would be delayed until after a meeting with Ms.Lofstrom's replacement had occurred.
PHS/EHD staff has discussed this site with the Central Valley Regional Water Quality Control Board staff.
There is no reason to meet and discuss further the requirements for this site. The only change in the
additional work needed is that instead of installing monitoring wells, the verification of the lateral and
vertical extent of soil and groundwater contamination can be shown by collecting and analyzing soil and
discrete grab groundwater samples from soil borings/probes. The following work is required for this site:
1) The domestic well(G-1)must be properly destroyed under permit.
2) A replacement well must be installed at the site. The replacement well will be installed to a
mutually agreed upon depth with a minimum grout seal of 200 feet per PHS/EHD guidelines.
3) Well construction details of G-1 must be provided to PHS/EHD before destruction of the well. If
construction details are not available, video logging of the well is required.
4) Three borings must be drilled with soil and groundwater samples collected to delineate the lateral
extent of currently known soil and groundwater contamination.
5) Two borings must be drilled with soil and groundwater samples collected to delineate the vertical
extent of soil and groundwater contamination.
6) A groundwater pumping test should be conducted as described in the"Revised Work Plan for
Aquifer Testing"dated October 20, 1999. Every attempt should be made to adjust the pumping
rate to maintain a consistent flow of groundwater without pumping the well dry. The pumping test
should be conducted at the next scheduled quarterly monitoring event in the presence of
PHS/EHD personnel.
7) The monitoring well GT-2 must be located.
A work plan detailing the above required work should be submitted to PHS/EHD by December 12,2001.
If you have any questions please contact Jeffrey Wong at(209)468-0335 or Margaret Lagorio at(209)
468-3449.
Donna Heran,REHS,Director
Environmental
Health Division
Jeffrey Wong, SeniorREHS Marg ,REHS
LOP/Site Mitigation Unit IV Supervisor
c: RWQCB,CentralValleyRegion—Marty Hartzell
c: Upgradient Environmental-Victor Cherven
A Division of San Joaquin County Health Care Scrvices
