Please submit a drilling permit application for each <br /> parcel with the $89.00 fee for each parcel to the EHD. Notify the EHD at least 72 <br /> hours before commencing each phase of fieldwork. <br /> To date, you have not complied with the EHD directive letter dated 17 June 2004 <br /> to destroy the domestic well (G-1). The deadline set by the EHD for this action <br /> has passed and you have not responded in any way to this directive. Please <br /> submit to the EHD by 15 October 2004 a written response with your intentions as <br /> to how you will meet this directive.
