Three boxes of UW lamps have been accumulating on site for several years according to the employee. A UW handler shall accumulate UW for no longer than one year from the date the UW was generated, or was received from another UW handler. A UW handler shall be able to demonstrate the length of time that the UW has been accumulated from the date it became a waste or was received. The UW handler may make this demonstration by: (1) Placing the UW in a container and marking or labeling the container with the earliest date that any UW in the container became a waste or was received. (2) Marking or labeling the individual item of UW with the date it became a waste or was received. (3) Maintaining an inventory system on site that identifies the date the UW being accumulated became a waste or was received. (4) Maintaining an inventory system on site that identifies the earliest date that any UW in a group of items of UW or group of containers of UW became a waste or was received,- (5) eceived;(5) Placing the UW in a specific accumulation area and marking or labeling the area to identify the earliest date that any UW in the area became a waste or was received; or (6) Any other method which clearly demonstrates the length of time that the UW has been accumulated from the date it became a waste or was received. The current UW lamps on site were marked with the accumulation start date of 7-28-11. Employee acknowledged that the UW needs to be disposed within one year of the accumulation start date. Employee acknowledged that the UW lamps need to be disposed before October 2, 2011. Submit a copy of the disposal record to this office by 10-2-11. An emergency coordinator and modified contingency plan information is lacking. There must be at least one emergency coordinator on site or on call to coordinate emergency response measures, and the following information must be posted by a phone: the name and phone number of the emergency coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; and the phone number of the fire department, unless the facility has a direct alarm. Employee appointed an emergency coordinator and posted the required information by a phone using the provided form. Corrected on site. 808. The UW lamps were not labeled. A UW handler shall label or mark UW to identify the type of waste. Lamps (i.e., each lamp), or a container in which the lamps are contained, shall be labeled or marked clearly with the following phrase: "Universal Waste-Lamp(s)". Employee labeled the boxes of UW lamps as "Universal Waste Lamps". Corrected on Site. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
