Three boxes of UW lamps have been accumulating on site for several years according to the <br /> employee. A UW handler shall accumulate UW for no longer than one year from the date the UW was <br /> generated, or was received from another UW handler. A UW handler shall be able to demonstrate the <br /> length of time that the UW has been accumulated from the date it became a waste or was received. The <br /> UW handler may make this demonstration by: <br /> (1) Placing the UW in a container and marking or labeling the container with the earliest date that any <br /> UW in the container became a waste or was received. <br /> (2) Marking or labeling the individual item of UW with the date it became a waste or was received. <br /> (3) Maintaining an inventory system on site that identifies the date the UW being accumulated became a <br /> waste or was received. <br /> (4) Maintaining an inventory system on site that identifies the earliest date that any UW in a group of <br /> items of UW or group of containers of UW became a waste or was received,- <br /> (5) <br /> eceived;(5) Placing the UW in a specific accumulation area and marking or labeling the area to identify the <br /> earliest date that any UW in the area became a waste or was received; or <br /> (6) Any other method which clearly demonstrates the length of time that the UW has been accumulated <br /> from the date it became a waste or was received. <br /> The current UW lamps on site were marked with the accumulation start date of 7-28-11. Employee <br /> acknowledged that the UW needs to be disposed within one year of the accumulation start date. <br /> Employee acknowledged that the UW lamps need to be disposed before October 2, 2011. Submit a <br /> copy of the disposal record to this office by 10-2-11. C\)) r <br /> 703. An emergency coordinator and modified contingency plan information is lacking. There must be at <br /> least one emergency coordinator on site or on call to coordinate emergency response measures, and <br /> the following information must be posted by a phone: the name and phone number of the emergency <br /> coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; and the <br /> phone number of the fire department, unless the facility has a direct alarm. Employee appointed an <br /> emergency coordinator and posted the required information by a phone using the provided form. <br /> Corrected on site. <br /> 808. The UW lamps were not labeled. A UW handler shall label or mark UW to identify the type of waste. <br /> Lamps (i.e., each lamp), or a container in which the lamps are contained, shall be labeled or marked <br /> clearly with the following phrase: "Universal Waste-Lamp(s)". Employee labeled the boxes of UW lamps <br /> as "Universal Waste Lamps".
