Do active or abandoned industrial, agricultural, domestic, or groundwater wells exist at the
treatment facility, or storage pond(s) area. If yes, provide a map showing their location(s).
Include available data on the depth of groundwater and information on aquifers and
aquitards identified beneath the property.
2. A narrative description of the wastewater conveyance, treatment, and disposal system at
the facility. Include a process flow diagram and a description of all sewage lift stations.
3. A description of the sources and types of wastewater flowing into the system, design flow
rates, and the design capacity of the system.
4. A description of emergency wastewater storage facilities or other means of preventing
system bypass or failure during reasonably foreseeable overload conditions (e.g.,power
failure or pipe blockage).
5. Chemical characterization of the potable water supply including pH, and total dissolved
solids. Discuss water softening and other water treatment processes that are needed or
typically used and the relationship between water supply treatment and wastewater
treatment plant effluent quality.
6. A description of the planned wastewater storage pond, including:
a. Surface area, total depth, and net volume capacity(at 2-foot freeboard level) for each
pond;
b. Operation and maintenance procedures;
c. Materials, thickness, and construction methods proposed for pond liner systems; and
d. Estimated hydraulic conductivity rate for pond liner system(s) as designed; and
e. An analysis of whether the waste in the pond has the potential to impact the underlying
groundwater quality.
7. Chemical characterization of treated effluent quality, including biochemical oxygen
demand, total suspended solids, total dissolved solids, nitrogenous compounds, and total
coliform organisms. This shall be based on data collected in the NPDES monitoring and
reporting program.
8. Narrative description of expected solids generation rates and handling/storage procedures:
a. Debris;
b. Grit and screenings; and
c. Biosolids;
d. Disposal procedures.
9. A description of the types of soil underlying the planned pond(s) (include a copy of the
geotechnical report if available).
