(ETIC)observed the destruction of one offsite
groundwater monitoring well for the above-referenced site. ETIC observed the destruction of well MW 11 at 1130
Monaco Court, Stockton, California on 12 October 2006, by Cascade Drilling Company, Inc. of Rancho Cordova,
California(C-57 license#717510). The well was destroyed by excavating around the well casing to a depth of 3 feet
below ground surface, and to a diameter of approximately 2 feet. The well casing was cut off at the bottom of the
excavation, leaving a 6-inch stub. The well was then pressure-grouted for 5 minutes at 30 pounds per square inch.
The excavation was then filled with clean soil and the surface was restored to match the surrounding surface. The
work was completed over a period of one day. Well destruction activities were observed by an inspector from the
San Joaquin County Environmental Health Department(SJCEHD).
The well debris and concrete generated during well destruction activities were contained in one 55-gallon drum The
waste was removed from the site on 28 November 2006 by Dillard Environmental Services and transported for
disposal to Republic Services Vasco Road Landfill in Livermore,California.
The former location of the well is shown in the attached site plan. Well construction details for the destroyed well are
summarized in the attached Department of Water Resources Form 188 (Well Completion Report) and the attached
boring log. Copies of the SJCEHD well destruction permit and waste documentation are attached.
If you have any questions or comments, please contact Ted Moise at (925) 602-4710, extension 23 or Bryan
Campbell at extension 24.
Sincerely, SotONAL Gk_0
BRYAN !GC/
Q CAMPBELL u'a
Ted Morse Bryan Campbell,P.G.#7724 _ No. 7724
Senior Project Manager Senior Geologist
JfT Exp. tY101
Attachments q P
cc: Ms.Jennifer Sedlachek,ExxonMobil Refining and Supply Company
CA1-F
Mr.James L.Barton,California Regional Water Quality Control Board,Central Valley Region
Mr.Bob Fay,Bank of Stockton Trust Department
2285 Morello Avenue, Pleasant Hill, CA 94523 • Phone: 925.602.4710 • Fax:925.602.4720 • License #624022
o: ja %73W2UBLIC WI I WELL DESTRUCTION IWSWeII rk Uion Letts ReponO-3942 Well Destmaw-Text hrc
